Flower Mound Lacrosse Association is excited to offer the 2011 Winter Lacrosse Camp to be held at Gerault Park Athletic Fields in Flower Mound (1200 Gerault Road) on December 9, 10 and 11 for youth boys and December 10 and 11 for high school boys. The FMLA coaching staff in conjunction with Brine/Warrior is offering an all star coaching line up, inclusive of:

Scott Rodgers- Team USA Gold Medal Winner and current Assistant Coach, Marquette University, Named the Most Outstanding Player of the 2010 NCAA Championship and a two-time All-America honoree. Selected by the Toronto Nationals in the second round (seventh overall pick) of the 2010 MLL Draft ... taken by the Minnesota Swarm in the second round (15th overall pick) of the 2010 NLL Draft.

Ken Clausen- University of Virginia Defender and 4 time Division 1 All American, University of Virginia; Team USA Gold Medal Winner; Drafted fifth overall by Denver in the 2010 MLL Draft... Selected 22nd overall by the NLL's Colorado Mammoth.

High School Schedule

December 10th and 11th 9:00 am – 12:00 pm

This camp is geared to High School players and designed to help each player increase their knowledge, skill level and confidence to reach the next level in their game.

Flower Mound Lacrosse Association - Player Waiver

Players Will NOT Be Allowed To Play Without Parents Signed Permission

In consideration of being allowed to participate in any way in Flower Mound Lacrosse Association, and related events and activities, the undersigned:

1. Agree that the parent(s) or legal guardian(s) will instruct the minor participant that prior to participation he or she should inspect the facilities and equipment to be used, and if the participant believes anything unsafe, he or she should immediately advise his or her coach or supervisor of such condition(s) and refuse to participate.

2. Acknowledge and fully understand that each participant will be engaging in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from their own actions, inactions, or negligence but the actions, inaction or negligence of others, the rules of play, or the condition of the promises or of any equipment used. Further, that there may be other risks not known to us or not reasonably foreseeable at this time.

3. Assume all the foregoing risk and accept personal responsibility for the damages following such injury, permanent disability or death.

4. Release, waive, discharge and covenant not sue Flower Mound Lacrosse Association, its respective administrators, directors, agents, coaches, and other employees of the organization, individual team coaches, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and leassors of premises used to conduct any activities or events, all of which are hereinafter referred to as "releasees", from any and all liability to each of the undersigned, his or her heirs and the next of kin for any and all claims, demands, losses or damages on account of injury, including death or damage to property, caused or alleged to be caused in whole or in part by the negligence of the releasees or otherwise.

5. In case of accident or illness or that emergency medical attention is deemed necessary, I authorize the calling of medical services, and I give the Flower Mound Lacrosse Association permission to provide first aid and/or transport to the nearest emergency medical facility. I also give permission for necessary emergency treatment by a physician and/or hospital/clinic. By my signature and my free will I do hereby agree to indemnify and hold harmless the Flower Mound Lacrosse Association from any and all claims and demands, cost or expense arising out of any injuries to me or any party I am responsible for, and that I am liable for all such cost incurred.
